The following data marked with an *will be utilized in making official name tags which must be displayed at all times. The remaining data is required for insurance, accounting procedures and notification in case of cancellation.

If you are a student, list Professor's or Teacher's Name: Teacher's of students under the age of 18 should have IN THEIR POSSESSION wriiten parental approval agreeing to the student's participation and emergency medical treatment if required.

These accommodations are rustic. Bathrooms and showers are located in a building separate from the bunkhouses. No linens are being furnished. Bring your own.
Tenting Option (within the group camp): per person

Note: There are no cooking facilities for groups to do their own in the vicinity of the group camp. If you bring your own food, select the kind that doesn't need cooking. However cooking fires are permitted in other areas of the park nearby. If you choose this option you must pay your camping fees at the park office instead of remitting it with this form. If you want to participate in the field meeting activities, you will still be required to register with this form. You will not need to pay an additional tent lodging option fee to OAS.

BEFORE September 19. Request cancellation and refund before that date . Non-refundable portion will be used to defray extra costs of postage, bank charges and administrative details incurred by cancellations.
